Contact Consent Form

I am interested in being contacted by a representative of the NJ Peer Recovery support program. You can reach me by telephone or email at the numbers below.

What you can expect when you reach out:

To immediately connect with a highly-trained peer support specialist who has been there

To be heard, understood, and supported in complete confidence

To get help creating a personalized plan for recovery and wellness that will work for you

To gain access to the local resources you will need to reach your goals

At some point, we will ask for your name and date of birth to verify your Medicaid status. But you will not be required to provide either. You are welcome to remain completely anonymous, and do not have to share any information you are not comfortable providing.

If you don’t have Medicaid and would like to sign up, we can connect you with the resources to do so.

*NJ Peer Recovery staff is not available on the following holidays; Thanksgiving, Black Friday, Christmas, New Years Day, MLK, Memorial Day, 4th of July and Labor Day. But you can always request a call or send us an email and a peer support specialist will get back to you as soon as possible.

We are not a crisis support hotline. If you or someone you know is in immediate danger of harming themselves or if you are experiencing a medical emergency, please call 911 or go to the nearest emergency room.
